This document describes the privacy policy of PM Dietary Services for the management of clients’ personal information. PM Dietary Services is bound by the Privacy Act 1988 (Commonwealth) and the applicable Privacy Principles in the Privacy Act when handling personal information, along with the Health Records Act 2001 (Vic) and the applicable Health Privacy Principles in the Health Records Act when handling personal information, which is health information.
Clients’ Personal Information
Your personal information is securely stored in passworded electronic files only authorised employees can access. Such information includes name, address, date of birth, Medicare number, billing and credit card details, telephone numbers and email addresses, medical history and further personal information collected during the provision of our nutrition services.
Personal Information Collection Methods
Your personal information is collected in a variety of ways by PM Dietary Services: the new client intake form (hard or soft copy), referral form for family/carers, email correspondence, minutes written during consultations, health practitioners’ referrals and laboratory analyses results.
Any health information collected from you or your health practitioner or other Allied Health service provider is accessed, stored and solely utilised to formulate individualised and appropriate dietary advice.
The provision of safe and accurate dietetics services is made possible by the collection of relevant Health information. If you do not wish us to access such information, we may be unable to provide optimal Medical Nutrition advice.
Consent
By registering details on our Website or by email or telephone, and/or using the Website or completing our Client Intake and Consent Form or otherwise engaging our dietetics services, you are giving PM Dietary Services your consent to collect, utilise and disclose personal information (including Health Information) for the sole purpose of providing medical nutrition advice.
Purpose Of Holding Personal Information
A client’s personal information is gathered and used for the purpose of providing dietetic services, which includes assessing, diagnosing and treating a client’s presenting issue. The personal information is retained in order to document what happens during sessions and enables the dietitian to provide a relevant and informed dietetics service.
Disclosure Of Personal Information
Clients’ personal information will not be disclosed except when:
It is subpoenaed by a court; or
Failure to disclose the information would in the reasonable belief of the dietitian place a client or another person at serious risk to life, health or safety; or
The client’s prior approval has been obtained to provide a written report to another professional, to discuss the material with another person, e.g. a parent, employer or health provider, to disclose the information in another way or when you would reasonably expect your personal information to be disclosed to another professional or agency (e.g. your GP) and disclosure of your personal information to that third party is for a purpose which is directly related to the primary purpose for which your personal information was collected. Disclosure may also be required or authorised by law.
